Dr. Carter is the ER's Chief Resident. He's affectionately regarded by his co-workers, who watched him mature from a naive medical student into a respected leader. He's worked hard, first pursuing a career in surgery, and then deciding that the ER was where he belonged.

After a violent attack by a patient left a medical student dead and seriously wounded him, Carter succumbed to substance abuse. Supportive co-workers helped him seek treatment. His cousin had been disabled by a drug overdose, so Dr. Carter was very motivated in his own recovery, with the help of continuing caring and support from his friends.

His parents are separated, and Carter often turns to his grandmother for strength in tough times. In recent years, he's nursed her through ill health and the death of her spouse. After Carter's brief flirtation with Susan Lewis ended, Carter realized his feelings for Abby, and their close relationship deepened into love.
